1. The undersigned hereby applies for Membership in the COUNCIL OF MARINE CARRIERS (CMC) as a TIER I / TIER II / TIER III / TIER IV:

2. The Applicant acknowledges that the undersigned will be bound by all the obligations of Membership in CMC included in the Bylaws. Additionally, the applicant acknowledges the mandate of the CMC is to protect the interests and safeguard the rights of its members and promote growth of the domestic marine transportation industry. Members are expected to conduct operations safely and ethically while respecting the environment in which we work. A copy of the bylaws is available on request.
3. The undersigned agrees that, if accepted as a member, the member will promptly pay the appropriate membership dues as invoiced by CMC. If accepted as a Tier I member, the member will furnish the details of the member’s Gross Seagoing Payroll as defined in Section 1 of the Bylaws, upon demand of the Council of Marine Carriers.
4. The undersigned hereby appoints the following persons as its representative and alternate representative pursuant to Section 2, Paragraph 2.02.2 of the bylaws.
